Discordant Findings Investigations: Roles and responsibilities of the laboratory, manufacturer and IQMH

What is root cause analysis?

Root cause analysis (RCA) is a methodology for finding the most important reason for a problem or occurrence, in order to mitigate risks of recurrence. RCA can be used in almost any situation where there is a gap between actual and desired performance.
