After a review of the discordant findings investigation (DFI) process, the ease of completing and effectiveness of the DFI were identified as areas for improvement; therefore, the handling of the A0 action code was revised.
Laboratories are expected to do a thorough investigation when they have a discordant finding action code in their survey performance assessment. Action to be taken includes investigation and performing a root cause analysis.
