The purpose of this bulletin is to remind participants of the changes related to the packaging and shipment process for Coagulation survey material in 2021.

A copy of the Shipment Changes for 2021 Coagulation Surveys was included in the previous September 2020 survey.

Packaging and Shipping

The Proficiency Testing (PT) material for 2021 for the following surveys will be shipped to your facility on Jan 25^th, 2021.

Depending on your laboratory’s license/enrolment you may receive some or all the survey material.

One package includes survey material for:

• COAG
• COAG-DD
• COAG-FA

A second package may be received with frozen survey material for:

• COAG-TH

On receipt, please forward the package(s) to the appropriate laboratory area(s) for storage and testing dates included in the package inserts.

• On January 25^th, 2021, you will receive a notification and an acknowledgement form available in QView™ for you to record the receipt date and condition of the packaging and samples.
• Samples must be tested according to the PT Testing Schedule, also available on iqmh.org website.
• A follow-up email will be sent to participants when the analysis worksheets are posted.
• More information regarding sample preparation, testing and reporting instructions are available on the analysis worksheets posted in your site-specific, password-protected QView™ homepage under “Active Forms and Worksheets.”
