Survey Participation
To ensure product availability, orders for 2025 proficiency testing programs should be placed no later than December 6, 2024 by using the online “Request a Quote” application below, or by sending an email to info@iqmh.org. After this date, due to the number of samples available, surveys are offered on a first-come, first-served basis.

New Surveys
Fetal Fibronectin (ENDO-fFN)
- Suitable for all laboratory instruments.
- Participants can expect to submit qualitative results, such as: positive and negative.
- Liquid material, reconstitution not required.
Flow Cytometry Educational (FLOW-E)
- A case-based educational survey illustrating classical, interesting, and/or challenging flow cytometry immunophenotyping, including novel immunophenotypic markers, panels, and/or gating techniques.
Modified Surveys
Chemistry Routine (CHEM)
- Sample volume change: 5.0 mL to 3.0 mL for 2025.
Fecal Occult Blood (CHEM-FOB)
- Sample frequency change: 3 surveys per year to 2 surveys per year for 2025.
- 3 × 2.0 mL samples will continue to be offered per survey.
Chemistry Urinalysis (CHEM-UDIP)
- Now includes urine hCG. Participants can choose from two packages:
- Package 1 – Report the full test menu (CHEM-UDIP)
- Package 2 – Report hCG only (CHEM-UDIP-hCG)
Endocrinology and Tumor Markers, Endocrinology Special, and Endocrinology Prostate Specific Antigen (ENDO-A, ENDO-B, ENDO-PSA)
- Sample quantity change: 4 samples per survey to 3 samples per survey for 2025 (ENDO-A, ENDO-B & ENDO-PSA).
- Sample volume change: 3.0 mL to 1.0 mL for 2025 (ENDO-PSA only)
Lymphocyte Subsets (FLOW-LS)
- The FLOW-HD survey has been revised to the assessment of normal lymphocyte subsets and relevant immunophenotypic markers in the context of whole blood control material.
Point-of-Care Testing – Complete Blood Cell Count and Leukocyte Differential (HEMA-POC)
- The following POCT groups will be moved from the HEMA-LD survey to the new HEMA-POC survey:
- Group 8 – HemoCue Hb 201+
- Group 11 & 11+ - CSAN® Pronto™
- Group 12 – Sight OLO
Pathology – Immunohistochemistry (PATH)
- Pathology – Immunohistochemistry (PATH)
- Sample frequency change: 4 surveys per year to 3 surveys per year for 2025.
Point-of-Care Testing Glucose (POCT-GL)
- A volume discount will be offered for orders of 3 or more sets.
Severe Acute Respiratory Syndrome Coronavirus 2 (VIRO-COV)
- Sample frequency change: 3 surveys per year to 2 surveys per year for 2025.
- Sample quantity change: 2 swabs per survey to 3 swabs per survey for 2025.
Discontinued Surveys
Bone Marrow Virtual Slide-based (BONE-VSB)
- Survey has been discontinued and a new format for evaluation of preparation and staining is under development.
Urine Human Chorionic Gonadotropin (CHEM-UCG)
- Survey has been discontinued.
- hCG will now be assessed in the CHEM-UDIP survey.
Leukocyte Immunophenotyping for Hematologic Disorders (FLOW-HD)
- Survey has been discontinued and replaced with FLOW-LS and FLOW-E.
Using the Quote Request Application
Review the available programs by selecting the discipline name from the list below. Click on the “Program Details” button to find detailed information regarding each program. Please review to ensure survey material compatibility with your analytical system before requesting a quote.
Requesting a quote is easy! Simply select the programs you are interested in and submit your quote request by clicking on the “Request Quote” button at the bottom of the screen. You will receive the quote by email within five business days. After you review the quote you must "accept" the quote online by clicking the "Accept" quote button. Once you accept the quote, your laboratory will be enrolled into the program and then you will receive an invoice. If you need assistance, please contact IQMH at info@iqmh.org.
