Historically, the Centre for Accreditation has used the term “error of fact” to define a non-conformance cited during an assessment visit that is incorrect because the evidence reviewed by the assessment team was misinterpreted. In order to align with an existing process at IQMH for Appeals and Complaints Investigation, we have decided to change the terminology from “error of fact” to “appeal”. The definition of an appeal, as it relates to the Centre for Accreditation, has two categories:
