Point-of-care (POC) compliance evaluation, whether for internal audit or IQMH accreditation assessment, is very challenging when there are different POC analytes and devices throughout a facility or multiple facilities.
With the help of IQMH Centre for Accreditation Assessors, a new tool has been developed to aid laboratory management and POC coordinators when preparing for internal audits and IQMH accreditation assessments. This tool will also benefit IQMH Accreditation Assessors as a complement to the ePortal checklist, in order to keep track of all the areas assessed for POC.
This tool was developed with the intention of addressing these top four challenging situations:
Issue 1: Using a single checklist for multiple devices/POC test
Assessing multiple devices or POC tests with a single audit checklist can be challenging. It’s difficult to keep track of the conformance for each device.
This tool can be used to keep track of the individual POC programs. The tool includes prompts to remind the user what topics are included within each subsection of the requirements. This will help to kick-start questions for each subsection and will provide a succinct roadmap to easily view progress. With this support tool, accuracy and time-management will be improved.
Issue 2: Tracking all components of the checklist
When assessing POC, the auditor must speak with the lab staff about all components of the checklist, but he or she must also confirm implementation and seek feedback from some users of the device. For example, when reviewing quality control practices, an assessor will review the documented process and procedures with laboratory staff. However, there is much to be gained by speaking with a user such as a nurse about their part of the quality control process.
Within the tool, each topic has been pre-set with two boxes for keeping track of evidence, one for lab staff (L) and one for the device users (U). All topics should be discussed with the laboratory staff, but if the topic does not require confirmation with a user, the user box has been greyed-out.
Issue 3: Support for horizontal and vertical auditing
Auditing can be performed two ways: horizontally or vertically.
An evaluation of one process or activity across several types of POC is a horizontal approach. A vertical audit is an evaluation of all the processes of a single type of POC. When speaking to lab staff in an organization with multiple POC analytes, the most likely approach is to address each process in turn, such as training, and discuss the various analytes as they pertain to that topic (horizontal). When speaking to a POC user, the conversation will likely focus on one type of test and will flow vertically down the various aspects. This can be challenging to visualize and manage with the IQMH ePortal checklist.
The POC assessment tool is laid out to naturally support horizontal and vertical auditing. It will be useful to apply the form horizontally when asking questions of laboratory staff, and vertically when asking questions of the users of POC testing. For example, when speaking to a laboratory POC testing manager about training, review how the training process relates to all types of POC, you can move horizontally across the form. When speaking to nursing staff about a specific test, apply the form vertically down the various aspects to assess their use of that test.
Issue 4: Managing paper work

The POC Assessment tool is an editable PDF that can be downloaded to an iPad or laptop. Using an application which allows editing of PDF forms, the assessment form can be completed and saved, and emailed to a user.
